The staff at Lake Region Urgent Care has been committed to offering high-quality urgent care for children and adults since 2010. As of September, 2020 Lake Region Urgent Care & Occupational Medicine became a Wayne Memorial Community Health Centers facility!
With our convenient daytime, evening and weekend hours and “no appointment” policy we are able to treat non-life threatening conditions such as lacerations, burns, fractures, sprains, colds and allergic reactions. Our team of physicians, Family Nurse Practitioners, Registered Nurses, phlebotomists, respiratory and x-ray techs work together to provide you with timely care—especially when an Emergency Department visit isn’t warranted and your primary care or pediatric office isn’t available to see you or your child.
IF YOU ARE EXPERIENCING A TRUE MEDICAL EMERGENCY SUCH AS CHEST PAIN, STROKE OR MAJOR INJURY, CALL 911 OR GO TO THE NEAREST HOSPITAL EMERGENCY ROOM.

WMCHC participates with many out-of-state insurance plans. However, there are some such as New York State Medicaid plans that refuse to contract with our organization. Those patients will have to pay out of pocket for services.
For your convenience and to expedite the check-in process, we have gathered documents including consents and registration forms required for patients to be seen at LRUC. Please print the forms located below and fill out all information. Upon arrival to the clinic, present these forms along with insurance cards and photo identification to the front desk.
