Every three years, Wayne Memorial Hospital (WMH) and Wayne Memorial Community Health Centers (WMCHC) ask the public to help them assess and answer our community’s health needs via an internet survey as well as one-on-one interviews with key stakeholders including patients, providers and community leaders. The most recent Community Health Needs Assessment was conducted in 2019. The Implementation Strategy, the final report and user guide as well as an executive summary approved by the Wayne Memorial Hospital Board of Trustees may be accessed by clicking on the links below:

STATEMENT FROM WMH CEO DAVID L. HOFF
“The importance of public input into surveys such as this cannot be underestimated,” stated David L. Hoff, chief executive officer, Wayne Memorial Hospital. Noting that the 2019 needs assessment is the hospital’s fourth since 2009 he added, “The feedback from prior needs surveys has proven very beneficial to our community. It’s contributed to the opening of new services, such as our cardiac catheterization laboratory, and opened new avenues for collaboration with organizations within our service area, including local government.”
