OCCUPATIONAL MEDICINE

Urgent care evaluation and treatment for conditions such as the following: allergic reactions; coughs, colds, fever, flu, sore throat and sinus infections; earache, minor cuts and burns; fractures; sprains and strains; upper respiratory infections; removal of foreign bodies; dehydration; nausea, vomiting and diarrhea; Urinary Tract Infections, back pain and more.

For your convenience and to expedite the check-in process, we have gathered documents including consents and registration forms required for patients to be seen at LRUC. Please print the forms located below and fill out all information. Upon arrival to the clinic, present these forms along with insurance cards and photo identification to the front desk.

We accept most major insurances, managed care plans and Medical Assistance. We also offer a sliding fee scale for those who qualify based on income. For specific information regarding billing, please call 607-637-5435, (570) 251-6641 or (570) 253-8226.
Insurance Plans Outside of Pennsylvania
WMCHC participates with many out-of-state insurance plans. However, there are some such as New York State Medicaid plans that refuse to contract with our organization. Those patients will have to pay out of pocket for services.
We offer telemedicine visits for the safety of our patients and staff. Please check with your insurance regarding coverage of telemedicine service. Patients who wish to pre-register to receive services either at the urgent care or via telemedicine, please complete the appropriate forms and click here to utilize our Patient Portal.
At Lake Regional Urgent Care and Occupational Medicine our team of physicians, advanced care practitioners and nurses offer a comprehensive worker’s health program including pre-employment screenings, work-related injury management and return to work assessments including disability determination. Our facility is designed with a separate entrance, registration and suite of treatment rooms utilized for Occupational Medicine services. We are contracted with several local and national companies to perform these services. Call your employer to ensure Lake Regional Urgent Care and Occupational Medicine is on their list of Worker’s Compensation medical facilities.

Our team of physicians, Family Nurse Practitioners, Registered Nurses, phlebotomists, respiratory and x-ray techs work together to provide you with timely care--especially when an Emergency Department visit isn't warranted and your primary care or pediatric office isn't available to see you or your child.
