Nurse Case Manager Appointed at WMCHC
Carollee Keating, RN, was recently appointed as the Case Management Registered Nurse as part of WMCHC’s membership with Keystone Accountable Care Organization (ACO) in April 2013.
According to the health centers’ administration, the addition of this position allows WMCHC to more closely evaluate care utilizing a team approach by holding multi-disciplinary staff meetings and discussing best patient outcomes.
In her new role, Keating will follow up with patients to keep appointments, conduct medication reviews and arrange additional services for those coping with complicated situations. She looks forward to “being a member of a collaborative team and helping patients understand that properly maintaining their healthcare objectives will lead to better quality of life.”
Keating is a graduate of Luzerne County Community College. She has been a registered nurse for more than 17 years. Her practical experience includes Telemetry, Neonatal Intensive Care, Hemodialysis and Wound Care.
