Clinical Care and Patient Wellness Coordinator

Description:
Are you looking to grow your career and have a positive impact on patients and colleagues across multiple sites? We are seeking a highly motivated energetic leader to serve in an expanded nursing role. Promote preventative patient care by overseeing and performing screening services. Coordinate and oversees clinical care for the entire center. Facilitates Wellness Program for CHC and works with external partners. Assists with development of pathways clinical guidelines and policy development and implementation related to clinical care of patients. Assists Quality Director and Medical Director in developing and overseeing clinical function across a four-county multi-specialty physician group. Full-time daytime hours with some flexibility and local travel required.

Requirements:
In addition to excellent clinical skills the right candidate will possess superior organizational critical thinking and communication skills along with proven customer-focused interpersonal skills and the ability to teach others. Graduate of an accredited nursing program (RN preferred LPN minimally required.) Current PA License required. Minimum of 3-5 years experience in clinical nursing case management or long-term care required. Minimum of 1 year of ambulatory care experience required.

Dental Assistant/Expanded Function Dental Assistant

Description:
Company growth has created openings for a Full and/or Part time Dental Assistant. Candidate will perform the functions of a Dental Assistant/EFDA under direct supervision of a dentist. Limited evening and weekend hours may be required. Current openings in Lords Valley and Honesdale offices. Requirements:
Dental Assistant: Must possess authorization from the State Board of Dentistry to perform dental radiographic procedures. Graduate of Dental Assisting program and experience preferred. Information Systems Manager

Description:
Information Systems Manager for progressive fast growing medical group. Primary duties include growing, implementing and managing various IT platforms in support of multispecialty, multi-site physician and dental practices. Heavy project management responsibilities, coordinating with internal and external contacts. Ensure quality and effective EHR programming development and implementation. Maintain system libraries and ensure proper database administration. Plan and ensure proper system version management for software and operating systems. Supervise IT Staff. Training oversight and implementation of all staff and providers for EMR utilization and workflows. Design and manage IT reporting systems for quality initiatives such as Meaningful Use and Patient Centered Medical Home.

Requirements:
At least 1 year experience in programming capacity or the equivalent education (to include knowledge and use of 2 of the following: SQL .NET C# or a relational database system); Four years experience in operating environments including Windows Server VMWARE Citrix Windows Clients; Understanding and practical experience in system administration file management and system library functions; Requires effective communication with end users as it relates to operations of the hospital information system; BS in computer science information systems or its equivalent in education is required; Previous healthcare physician practice management EMR systems experience is preferred.

Physician-Primary Care

Description:
Family Practice or Internal Medicine-Outpatient Wayne Memorial Community Health Centers is a Federally Qualified Health Center (FQHC). Our position qualifies for loan repayment and waiver positions. Our website is @ WMH.org. Click on the Green Radio button at the top right to see the health center group. There is a primary care tab. We are recruiting for both the Honesdale and the Pike County offices. Click on the Primary care tab. We are a progressive multi specialty group with nine medical sites one dental and one behavioral office in northeastern Pennsylvania. We are affiliated with Wayne Memorial Hospital. Our primary care practices are outpatient only business hours with one evening per week and one to two weekend (1/2 day shifts) per month. Call is limited to phone support. Inpatient service is provided by the systems hospitalist group. Candidates can follow their own inpatients if desired. EMR platform is GE Centricity which is fully integrated with the hospital system. Practice is in the process of seeking level III medical home certification.

Requirements:
Candidate must have M.D. D.O. or foreign degree equivalent; have completed residency in family or internal medicine and be board certified or board eligible. Reply by resume to Frederick Jackson Executive Director.

Registered Nurse Walk-in Center

Description:
Registered Nurse for walk-in urgent care center to provide clinical care in office setting. Duties include: • Clinical Assistance to physicians • Triage of potential patients to determine appropriateness for walk-in center or referral to emergency room • Responsible for training and oversight of medical assistants • Help develop an implement clinical protocols and procedure

Requirements:
