Together for Health Family Dental Center Our community-based center provides primary care dental services to Wayne and Pike county residents 21 years of age or younger. The Together for Health Family Dental Center is equipped with six full operatories and provides prevention, treatment, education, periodontal and restorative procedures. The staff includes one fulltime and two part-time dentists along with one dental hygienist, two dental assistants and two medical secretaries. The Together for Health Family Dental Center accepts Pennsylvania Medical Assistance (ACCESS) Cards, PA Blue CHIP, and most major dental insurance plans. A sliding fee scale payment plan is available for uninsured and under-insured individuals. HOURS: Monday through Friday 8am to 4pm The Together for Health Family Dental Center is located at the Stourbridge Mall at Route 6 and Maple Avenue in Honesdale. For appointments, please call (570) 251-6534. The Together for Health Family Dental Center is made possible through funding received from the Pennsylvania Department of Health, the Wayne Memorial Hospital Auxiliary and the Wayne Memorial Health System. The Together for Health Family Dental Center will not deny emergency services to any person residing in the facility's service area on the grounds that the person is unable to pay for those services. Services will be provided without regard to race, color, national origin or creed. The Facility's service area is Wayne and Pike Counties in Pennsylvania. This project is funded, in part, under a contract with the Pennsylvania Department of Health. The Pennsylvania Department of Health, Harrisburg, Pennsylvania, supplied basic data for use in this study.
